Authorize a contract for a fully insured vision plan for Austin Human Resources with Avatar Holdings, LLC/Avesis Third Party Administrators, LLC, or one of the other qualified offerors, for an initial term of three years with up to two one-year extension options in an amount not to exceed $8,200,000. Funding: $1,200,000 is contingent upon approval of the Employee Benefits Fund. Funding for the remaining contract term is contingent upon available funding in future budgets.
