Authorize negotiation and execution of Amendment No. 12 to the agreement with the Asian American Resource Center, Inc. d/b/a Austin Asian Community Health Initiative for the Community Health Navigator program to add a 12-month extension option ending on September 30, 2025 and add funding in an amount not to exceed $587,305, for a revised total agreement amount not to exceed $4,103,910.
